В данном обзоре представлен системный анализ современных данных о молекулярно-генетических основах ожирения, выполненный на основе поиска и анализа литературы по базам данных Scopus, Web of Science, PubMed, The Cochrane Library, РИНЦ. Рассматривается вклад ключевых генов в развитие метаболических нарушений и оцениваются возможности применения генетического тестирования для персонализации профилактических мероприятий. Особое внимание уделено анализу работы образовательного кластера «Здоровое питание» как инновационной модели интеграции научных исследований, образовательных программ и практического здравоохранения. На основании комплексного анализа данных предложены практические алгоритмы персонализированной профилактики с использованием генетического тестирования, проведена оценка эффективности цифровых образовательных платформ, детально рассмотрены организационные и экономические аспекты внедрения кластерной модели в систему российского здравоохранения. Особое значение придаётся разработке практических рекомендаций для внедрения персонализированного подхода в клиническую практику и систему общественного здравоохранения. Полученные результаты демонстрируют высокую перспективность образовательных кластеров для совершенствования системы профилактики ожирения и могут быть использованы при разработке целевых региональных программ укрепления общественного здоровья, что будет способствовать снижению заболеваемости и улучшению качества жизни населения.</p><sec><title>Участие авторов</title><p>Участие авторов: Сошина М.С., Лапик И.А. — сбор и обработка материала, написание текста, редактирование, составление списка литературы; Гаппарова К.М., Тармаева И.Ю. — концепция и дизайн исследования, редактирование текста, утверждение окончательного варианта статьи. This review presents a systematic analysis of current data on the molecular and genetic basis of obesity, based on literature search and analysis in Scopus, Web of Science, PubMed, The Cochrane Library, and RSCI databases. The contribution of key genes to the development of metabolic disorders is examined, and the potential applications of genetic testing for personalizing preventive measures are evaluated. Special attention is paid to the analysis of the “Healthy Nutrition” educational cluster as an innovative model integrating scientific research, educational programs, and practical healthcare. Based on a comprehensive data analysis, practical algorithms for personalized prevention using genetic testing are proposed, the effectiveness of digital educational platforms is assessed, and organizational and economic aspects of implementing the cluster model in the Russian healthcare system are examined in detail. Particular importance is given to the development of practical recommendations for implementing a personalized approach in clinical practice and the public healthcare system. В связи с этим особую актуальность приобретает профилактика ожирения.</p><p>Профилактика ожирения подразделяется на три уровня: первичную (предотвращение у здоровых лиц через популяризацию здорового питания и физической активности), вторичную (раннее выявление избыточного веса и индивидуальные программы коррекции с учётом психологических аспектов) и третичную (мультидисциплинарный подход для предотвращения осложнений у пациентов с установленным диагнозом). Традиционные подходы к профилактике часто не учитывают индивидуальные генетические риски, что снижает её эффективность. Внедрение молекулярно-генетических исследований позволяет разрабатывать персонализированные программы профилактики, основанные на понимании патогенетических механизмов.</p><p>Цель статьи — обосновать необходимость внедрения научно-практических программ в систему здравоохранения РФ, включая создание образовательных кластеров для реализации комплексных программ гигиенического воспитания населения.</p><p>Роль генетических факторов в профилактике ожирения</p><p>Генетическая предрасположенность играет ключевую роль в развитии ожирения, и понимание молекулярных механизмов действия генов FTO, MC4R, ADRB2, PPARG, TCF7L2, CD36, BCMO1, VDR, ALPL, FUT2, MTHFR позволяет разрабатывать персонализированные программы профилактики.</p><p>Например, ген FTO (Fat mass and obesity-associated protein) является одним из наиболее изученных генов, ассоциированных с ожирением. Полиморфизмы FTO связаны с повышенным риском ожирения за счёт увеличения потребления высококалорийной пищи и снижения расхода энергии [<xref ref-type="bibr" rid="cit2">2</xref>]. Профилактические меры для носителей аллелей риска включают контроль размера порций и снижение потребления высококалорийных продуктов [<xref ref-type="bibr" rid="cit3">3</xref>], регулярную физическую активность для компенсации сниженного энергетического обмена [<xref ref-type="bibr" rid="cit4">4</xref>], а также обучение принципам осознанного питания для снижения риска переедания [<xref ref-type="bibr" rid="cit5">5</xref>].</p><p>Ген MC4R (Melanocortin 4 receptor) играет ключевую роль в регуляции аппетита и энергетического баланса. Мутации в этом гене приводят к гиперфагии и снижению расхода энергии [<xref ref-type="bibr" rid="cit6">6</xref>]. Профилактика для носителей мутаций MC4R включает строгий режим питания с фиксированным количеством приёмов пищи [<xref ref-type="bibr" rid="cit7">7</xref>], повышенную физическую активность для стимуляции энергозатрат [<xref ref-type="bibr" rid="cit8">8</xref>], раннее обучение принципам здорового образа жизни, особенно у детей с отягощённой наследственностью [<xref ref-type="bibr" rid="cit9">9</xref>].</p><p>Ген ADRB2 (β2-адренорецептор) влияет на липолиз и энергетический обмен. Полиморфизм Arg16Gly (rs1042713) связан со сниженным липолизом при физических нагрузках [<xref ref-type="bibr" rid="cit10">10</xref>]. Профилактические стратегии направлены на применение высокоинтенсивных интервальных тренировок для усиления липолиза [<xref ref-type="bibr" rid="cit11">11</xref>], диеты с контролируемым содержанием жиров и углеводов, а также регулярного мониторинга состава тела для оценки эффективности профилактических мер [12, 13].</p><p>Ген PPARG (Peroxisome proliferator-activated receptor gamma) регулирует дифференцировку адипоцитов и метаболизм липидов. Полиморфизм Pro12Ala (rs1801282) ассоциирован с повышенным накоплением висцерального жира [<xref ref-type="bibr" rid="cit14">14</xref>]. Профилактика для носителей этого полиморфизма включает диету с ограничением жиров для снижения риска накопления висцерального жира [<xref ref-type="bibr" rid="cit15">15</xref>], регулярные аэробные нагрузки для улучшения липидного обмена [<xref ref-type="bibr" rid="cit16">16</xref>], скрининг на инсулинорезистентность и другие метаболические нарушения [<xref ref-type="bibr" rid="cit17">17</xref>].</p><p>Ген TCF7L2 (Transcription factor 7-like 2) влияет на секрецию инсулина и метаболизм глюкозы. Полиморфизм rs7903146 увеличивает риск ожирения и сахарного диабета 2-го типа [<xref ref-type="bibr" rid="cit18">18</xref>]. Профилактические меры предусматривают диету с низким гликемическим индексом для контроля уровня глюкозы [<xref ref-type="bibr" rid="cit19">19</xref>], регулярные физические нагрузки для улучшения чувствительности к инсулину [<xref ref-type="bibr" rid="cit20">20</xref>], обучение принципам профилактики метаболического синдрома [<xref ref-type="bibr" rid="cit21">21</xref>].</p><p>Ген CD36 (fatty acid translocase) участвует в транспорте жирных кислот. Полиморфизм rs1761667 связан с повышенным предпочтением жирной пищи [<xref ref-type="bibr" rid="cit22">22</xref>]. Профилактика включает ограничение потребления насыщенных жиров [<xref ref-type="bibr" rid="cit23">23</xref>], обучение принципам сбалансированного питания [<xref ref-type="bibr" rid="cit24">24</xref>], использование функциональных продуктов с высоким содержанием полиненасыщенных жирных кислот [<xref ref-type="bibr" rid="cit25">25</xref>].</p><p>Ген BCMO1 (Beta-carotene-15,15’-monooxygenase) преобразует β-каротин в витамин А. Полиморфизм rs7501331 нарушает метаболизм витамина А, что может способствовать ожирению [<xref ref-type="bibr" rid="cit26">26</xref>]. Профилактические мероприятия направлены на контроль уровня витамина А в крови [<xref ref-type="bibr" rid="cit27">27</xref>], диету, богатую источниками готового витамина А (печень, рыбий жир) [<xref ref-type="bibr" rid="cit28">28</xref>], исключение избыточного потребления β-каротина у носителей полиморфизма [<xref ref-type="bibr" rid="cit29">29</xref>].</p><p>Ген VDR (Vitamin D receptor) влияет на метаболизм витамина D и минерализацию костей. Профилактика включает контроль уровня витамина D, достаточное потребление кальция и фосфора, умеренную инсоляцию [30, 31].</p><p>Ген ALPL (Alkaline phosphatase, tissue-nonspecific isoenzyme) отвечает за синтез щелочной фосфатазы, участвующей в минерализации костей и метаболизме витамина B6. Профилактика: мониторинг уровня щелочной фосфатазы, коррекция диеты с увеличением фосфора и кальция [32, 33].</p><p>Ген FUT2 (Fucosyltransferase-2) влияет на синтез антигенов группы крови и состав кишечной микробиоты. Профилактика: персонализированная диета [34, 35].</p><p>Ген MTHFR (Methylene tetrahydrofolate reductase) участвует в метаболизме фолатов, влияя на уровень гомоцистеина. Мутация C677T повышает риск гипергомоцистеинемии [<xref ref-type="bibr" rid="cit36">36</xref>]. Профилактика: контроль гомоцистеина и липидного спектра для предупреждения сердечно-сосудистых осложнений [<xref ref-type="bibr" rid="cit37">37</xref>].</p><p>Таким образом, интеграция генетического тестирования в программы профилактики ожирения позволяет разрабатывать персонализированные стратегии, направленные на коррекцию образа жизни. Учёт генетических особенностей даёт возможность предотвратить развитие ожирения у лиц с высокой предрасположенностью, снижая тем самым нагрузку на систему здравоохранения.</p><p>Особенности существующих программ профилактики ожирения в России</p><p>Современные программы профилактики ожирения в России имеют системные ограничения: отсутствие персонализированного подхода, ориентацию на общие рекомендации² и низкую осведомлённость населения о роли генетических факторов. Важной проблемой остаётся низкая осведомлённость населения о роли генетических факторов в развитии ожирения: лишь 12–15% россиян связывают заболевание с наследственностью при доказанном вкладе генетики в 40–70% случаев [<xref ref-type="bibr" rid="cit38">38</xref>], что снижает приверженность профилактическим мероприятиям и лечению из-за стигматизирующих представлений об ожирении как результате исключительно поведенческих факторов.</p><p>Особого внимания заслуживает дефицит практических школ для пациентов с ожирением. В отличие от многих европейских стран, где успешно функционируют национальные сети «школ ожирения», в России подобные инициативы не имеют системного распространения.</p><p>Наиболее тревожной представляется ситуация с детским ожирением. Около 18% российских школьников уже имеют избыточную массу тела, при этом профилактических программ для этой возрастной группы явно недостаточно [<xref ref-type="bibr" rid="cit39">39</xref>].</p><p>Образовательный кластер как эффективный инструмент гигиенического воспитания населения России</p><p>Образовательный кластер «Здоровое питание» создан на базе ФГБУН «ФИЦ питания и биотехнологии» как инструмент интеграции науки, образования и профилактики [40, 41]. Данный кластер — это современный подход к интеграции науки, образования и профилактики в области нутрициологии. Кластер объединяет 5 профильных кафедр: гигиены питания и токсикологии (Первый МГМУ им. И.М. Сеченова), диетологии и нутрициологии (РМАНПО), гастроэнтерологии и диетологии (РНИМУ им. Н.И. Пирогова), экологии и безопасности пищи (РУДН им. П. Лумумбы), а также научные ресурсы филиалов ФГБУН «ФИЦ питания и биотехнологии» (НИИ пищеконцентратной промышленности, НИИ детского питания, ВНИИ пищевой биотехнологии, Бирюлевский завод, Клиника лечебного питания).</p><p>В настоящее время кластер «Здоровое питание» реализует несколько направлений: информационное, образовательное, координационное и педагогическое [<xref ref-type="bibr" rid="cit42">42</xref>].</p><p>В рамках реализации стратегического проекта «Современная образовательная среда в Российской Федерации» были созданы электронные образовательные ресурсы, направленные на популяризацию здорового питания [43, 44], в том числе интерактивные форматы, сочетающие теоретическую информацию с практическим тренингом пищевого поведения. Дальнейшее развитие кластерной модели видится в следующих направлениях: расширение географического охвата за счёт создания региональных филиалов, углубление персонализации подходов с учётом генетических особенностей и возрастных потребностей, активное внедрение цифровых технологий, включая решения на основе искусственного интеллекта.</p><p>Комплексные программы здорового образа жизни как эффективный инструмент гигиенического воспитания населения России</p><p>Основные детерминанты ожирения: нарушение структуры питания, низкая физическая активность, хронический стресс. Формирование здоровых пищевых привычек необходимо начинать в раннем возрасте через интеграцию образовательных модулей в программы дошкольных и школьных учреждений.</p><p>Важное значение для формирования здорового образа жизни играет создание комплексных программ в качестве эффективного инструмента гигиенического воспитания населения России (рисунок).</p><p>Современные программы здорового образа жизни должны базироваться на трех фундаментальных принципах: научной обоснованности, индивидуальном подходе и практической направленности. Эти принципы реализуются через комплексную систему, включающую образовательный, практический и цифровой компоненты. Программа включает три блока: образовательный (основы рационального питания, гигиена продуктов), практический (мастер-классы, составление рационов, тренинги по самоконтролю) и цифровой (мобильные приложения, телемедицина, онлайн-обучение). Современные цифровые технологии играют ключевую роль в поддержке программы, предлагая участникам мобильные приложения для отслеживания показателей здоровья, системы телемедицинского консультирования и онлайн-платформы для дистанционного обучения. Эти решения особенно важны для обеспечения доступности программы в удалённых регионах страны. Реализация комплексной программы здорового образа жизни будет способствовать повышению медицинской грамотности населения, формированию устойчивых здоровых привычек и снижению распространённости алиментарно-зависимых заболеваний.</p><p>Заключение</p><p>Образовательный кластер «Здоровое питание» на базе ФГБУН «ФИЦ питания и биотехнологии» доказал эффективность как инструмент интеграции науки, образования и практики, обеспечивая персонализированный подход к профилактике на основе генетического тестирования. Реализация предложенных направлений развития образовательного кластера «Здоровое питание» будет способствовать достижению ключевых показателей государственной политики в области здорового долголетия и активного образа жизни.
