Введение. Инсульты остаются одной из главных причин инвалидизации и смертности населения России. Активно изучается влияние социально-экономических факторов на состояние здоровья населения. В этой связи актуально исследование взаимосвязи этих показателей в дотационных регионах, к которым относится Смоленская область, где показатели заболеваемости инсультами выше, чем в целом по стране.

Цель исследования — оценить взаимосвязь заболеваемости населения Смоленской области инсультами с социально-экономическими факторами и показателями состояния системы здравоохранения региона.

Материал и методы

Материал и методы. Методами непараметрической статистики проанализирована официальная статистическая информация, проведён опрос пациентов с инсультами для установления их материального благосостояния.

Результаты

Результаты. Отмечается ежегодное повышение заболеваемости инфарктами мозга и внутримозговыми кровоизлияниями, снижение заболеваемости недифференцированными инсультами, снижение обеспеченности врачами всех специальностей, а также неврологами, терапевтами и врачами скорой помощи, рост обеспеченности врачами общей практики. Установлена взаимосвязь заболеваемости инсультами с фактическим конечным потреблением домашних хозяйств, отношением среднемесячной номинальной начисленной заработной платы работников организаций к величине прожиточного минимума трудоспособного населения, дефицитом денежных доходов от общего объёма денежных доходов населения, индексом потребительских цен на товары и услуги, индексом Джини, средним размером начисленных пенсий, среднедушевыми денежными доходами населения, интегральным показателем качества жизни, обеспеченностью врачами общей практики, врачами-терапевтами, врачами скорой помощи.

Ограничения исследования

Ограничения исследования. Исследование выполнено на основе сводной агрегированной информации без использования первичных данных, результаты выявляют статистические, а не причинно-следственные связи, справедливы только для территории Смоленской области и сопоставимы с результатами аналогичных работ.

Выводы

Выводы. Наибольшее влияние на уровень заболеваемости инсультами в Смоленской области оказывает дефицит и неравенство распределения денежных доходов, низкий уровень качества жизни людей, кадровый дефицит и недостаточный объём профилактической работы амбулаторно-поликлинических учреждений.

Соблюдение этических стандартов. Протокол исследования одобрен независимым Этическим комитетом ФГБОУ ВО «Смоленский государственный медицинский университет» Министерства здравоохранения Российской Федерации (протокол № 7 от 04.12.2020). Каждый участник исследования дал информированное добровольное письменное согласие на участие в исследовании и публикацию персональной медицинской информации в обезличенной форме в журнале «Здравоохранение Российской Федерации».

Introduction

Introduction. The stroke was the leading reason for disability and mortality of the Russian population. In recent years, the influence of socio-economic factors on the health of the population has been actively studied. In this regard, it is important to study the relationship of these indicators in the subsidized region, which includes the Smolensk region, where the incidence of stroke is higher than in the whole country.

The purpose of the investigation is to assess the relationship between the incidence of stroke in the Smolensk region and socio-economic factors and indicators of the health system in the region.

Materials and methods

Materials and methods. Using the methods of nonparametric statistics, the official statistical information was analyzed as trend for 10 years. A survey of stroke patients was conducted to determine their material wealth.

Results

Results. There is an annual gain in the incidence of brain infarctions and intracerebral hemorrhages, a decline in the incidence of undifferentiated strokes, a decrease in the availability of doctors of all specialties, as well as neurologists, internists and emergency physicians, and an increase in the availability of general practitioners. The relationship between the incidence of the stroke and the actual final consumption of households, the ratio of the average monthly nominal accrued wages of employees of institutions to the subsistence minimum of the working-age population, the deficit of monetary income from the total monetary income of the population, the consumer price index for goods and services, the Gini index, the average amount of accrued pensions, the average per capita monetary income of the population, the integral indicator of the quality of life, the availability of general practitioners, therapists, emergency physicians.

Limitations

Limitations. The study was carried out on the basis of generalized aggregated information without any primary data, the results reveal statistical rather than causal relationships, and are valid only for the territory of the Central Federal District and are comparable with the results of similar work.

Conclusions

Conclusions: the greatest impact on the level of stroke incidence in the Smolensk region is caused by the deficit and inequality in the distribution of monetary income, the low level of quality of life of people, the personnel shortage and the insufficient volume of preventive work of outpatient clinics.

The protocol of the study was approved by the Independent Ethics Committee of the Smolensk State Medical University of the Ministry of Health of the Russian Federation (Protocol No. 7 of 04.12.2020). Each participant of the study gave informed voluntary written consent to participate in the study and publish personal medical information in an impersonal form in the journal "Health Care of the Russian Federation".
