Неравномерность развития материально-технической базы медицинских учреждений и ресурсного обеспечения муниципальных образований наблюдается во всём мире. В то же время ресурсное обеспечение системы здравоохранения связано с территориальными особенностями смертности населения. В целях снижения уровня смертности необходимо более глубокое понимание этой связи.</p><p>Цель исследования — оценка влияния ресурсного обеспечения здравоохранения на смертность населения с учётом иерархической вложенности муниципальных образований в имеющие различное финансирование здравоохранения и демографические показатели субъекты РФ.</p></sec><sec><title>Материал и методы</title><p>Материал и методы. Методом иерархического линейного моделирования проведён анализ материалов территориальных органов Федеральной службы государственной статистики и Единой межведомственной информационно-статистической системы по 265 муниципальным образованиям 6 субъектов РФ.</p></sec><sec><title>Результаты</title><p>Результаты. В результате моделирования определено, что на снижении смертности населения положительно сказывается обеспеченность ресурсами здравоохранения (врачи, медицинский персонал, койко-места) на уровне муниципальных образований при неоднозначном влиянии фактической стоимости территориальной программы ОМС на региональном уровне. </p></sec><sec><title>Заключение</title><p>Заключение. Полученные результаты соответствуют данным исследований региональной дифференциации смертности населения и ресурсного обеспечения здравоохранения, но позволяют определить влияние факторов с учётом уровня их формирования (региональный, муниципальный). Предлагаемые модели позволяют повысить качество принятия управленческих решений в системе здравоохранения, т.к. с учётом иерархической вложенности они разделяют влияние региональных и местных факторов на вариацию муниципальных образований по уровню смертности населения.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Introduction</title><p>Introduction. The uneven development of the medical material and technical base and resources is observed worldwide. At the same time, healthcare resource availability is associated with the territorial characteristics of the population’s mortality rate. In order to reduce mortality, a better understanding of this relationship is needed.</p><p>The purpose of the study is to assess the impact of healthcare resource availability on mortality, taking into account the hierarchical nesting of municipalities in subjects of the Russian Federation with further funding for health care and demographic indicators.</p></sec><sec><title>Material and methods</title><p>Material and methods. For these purposes, hierarchical linear modelling is used. The assessment was carried out on the data of 265 municipalities attributed to 6 constituent entities of the Russian Federation. The data sources are the Territorial Bodies of the Federal State Statistics Service and the Unified Interdepartmental Information and Statistical System (www.fedstat.ru). </p></sec><sec><title>Results</title><p>Results. As a result of modelling, the health care resources (doctors, medical personnel, beds) at the municipal level were determined to reduce the population mortality rate positively. At the same time, an ambiguous influence of the actual cost of the territorial compulsory medical insurance program was revealed at the regional level.</p></sec><sec><title>Conclusion</title><p>Conclusion. The results obtained correspond to studies devoted to the regional diversity of the population mortality rate and the available healthcare resources. However, they make it possible to determine the influence of factors taking into account the level of their formation (regional, municipal).
