Лабораторные информационные системы (ЛИС) становятся элементом работы лечебно-профилактических учреждений, обеспечивая высокое качество и эффективность клинической деятельности. </p><p>Цель исследования – анализ эффективности работы лабораторно-диагностической службы кардиохирургического стационара посредством контроля повторных назначений лабораторных исследований с оценкой экономической составляющей процесса.</p></sec><sec><title>Материал и методы</title><p>Материал и методы. Проведено исследование с историческим контролем обусловленности назначений лабораторных исследований до и после внедрения ЛИС в ФГБУ «НМИЦ сердечно-сосудистой хирургии им. А.Н. Бакулева». Ежеквартально в течение 2018 г. проводился анализ повторных назначений лабораторных исследований. Необоснованными признавались исследования, назначенные более 1 раза за 48 ч при отсутствии клинических показаний. Результаты представлены с помощью типовых методов описательной статистики.</p></sec><sec><title>Результаты</title><p>Результаты. С помощью данных ЛИС оценены частота лабораторных исследований различных категорий и показания к их назначению. Ежеквартально формировался план действий по устранению причин нерационального подхода к лабораторной диагностике: составление стандартных операционных процедур (СОП), проведение обучающих лекций и семинаров, направленных на уменьшение ошибок в преаналитическом этапе, организация тренингов и проверок знаний СОП. После проведенной работы в IV квартале 2018 г. отмечено снижение общего количества непрофильных исследований на 20%, а также уменьшение числа исследований в перерасчете на 1 пациента.</p></sec><sec><title>Заключение</title><p>Заключение. ЛИС стала важным инструментом для количественного и качественного анализа обоснованности назначения лабораторных исследований, а также для экономической оценки. Аналитические данные послужили основой для разработки лабораторных профилей, позволяющих продолжить оптимизацию данного направления и реализацию бережливых технологий в клинике. Полученные данные дали возможность оценить применение клинических рекомендаций по ведению пациентов с различной патологией. Выявлены наиболее скомпрометированные этапы лабораторного процесса.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Introduction</title><p>Introduction. In recent years, laboratory information systems (LIS) have become an element of medical institutions’ work, providing high quality and efficiency of clinical activities. Laboratory diagnosis plays a vital role in treating and preventing all known diseases, which requires compliance with specific quality criteria.</p></sec><sec><title>Material and methods</title><p>Material and methods. A study was conducted with the historical control of the laboratory tests’ conditiona-lity before and after introducing LIS in the A.N. Bakulev National Medical Research Center of Cardiovascular Surgery. During 2018, quarterly an analysis of the reappointments of the following groups of laboratory tests was carried out: hematological test, immunochemical analysis, biochemical blood test, hemostasis system (coagulogram), platelet aggregation with ADP. Unfounded recognized studies assigned more than once for 48 hours in the absence of clinical justification. Results are provided using standard descriptive statistics methods.</p></sec><sec><title>Results</title><p>Results. After the work carried out in the fourth quarter, there was a decrease in the total number of appointments by 20% and a decrease in studies performed in terms of one patient.</p></sec><sec><title>Conclusion</title><p>Conclusion. The introduction of LIS made it possible to obtain a tool for quantitative and qualitative analysis of the validity of the purpose of laboratory research and conduct an economic analysis of the results. The data obtained were subsequently used to develop laboratory profiles, allowing to continue the optimization of this area and the implementation of lean technologies in the clinic. Also, the data obtained made it possible to evaluate the knowledge and application of clinical recommendations to manage patients with various pathologies.
