Ключевой задачей Московского референс-центра лучевой диагностики (МРЦ) является оптимизация обработки исследований за счёт удалённого описания командой высококвалифицированных медицинских специалистов.</p><p>Цель исследования — оценить эффективность централизации лучевой диагностики в системе столичного здравоохранения. </p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. Проанализированы результаты применения телемедицинских технологий для проведения консультаций по результатам лучевых исследований в рамках работы МРЦ за период с августа 2020 г. по октябрь 2022 г.</p></sec><sec><title>Результаты</title><p>Результаты. Специалистами МРЦ описано 4 200 081 исследование (84,1% первичных описаний и 15,9% описаний «второе чтение»). По большинству модальностей наблюдается положительная динамика числа первичных описаний, коррелирующая с числом врачей в МРЦ для всех модальностей, кроме компьютерной томографии. Отмечен тренд на снижение времени описания для магнитно-резонансных исследований, флюорографий, денситометрии и рентгенографий. Аудит среди работников МРЦ показал достоверно большее число полного соответствия заключений, снижение числа замечаний общего характера и клинически значимых расхождений в сравнении с аудитом работы врачей на местах в медицинских организациях.</p></sec><sec><title>Ограничения исследования</title><p>Ограничения исследования. Отсутствие данных по анатомическим областям исследований, балансу классов заключений «норма/патология» и числу исследований с контрастным усилением не позволяет установить предпосылки для динамики времени описания исследований.</p></sec><sec><title>Заключение</title><p>Заключение. Централизация лучевой диагностики на примере МРЦ позволила улучшить качество работы медицинских работников и сократить время описания исследований.</p><p>Соблюдение этических стандартов. Авторы декларируют отсутствие явных и потенциальных конфликтов интересов в связи с публикацией данной статьи.</p></sec><sec><title>Поступила 10</title><p>Поступила 10.02.2023 / Принята к печати 26.04.2023 / Опубликована 29.04.2024</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Introduction</title><p>Introduction: the main goal of the Moscow Reference Center (MRC) for Diagnostic Radiology is to optimize study reporting by using teleradiology with a team of highly qualified specialists.</p></sec><sec><title>Purpose</title><p>Purpose: to evaluate the effectiveness of diagnostic radiology centralization in the capital health care system.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. We analyzed the results of telemedicine-based MRC consultations in diagnostic radiology examinations between 08.2020 and 10.2022.</p></sec><sec><title>Results</title><p>Results. The radiologists of MRC reported four million two hundred thousand eighty one study (84.1% first reading, and 15.9% second reading). For the majority of modalities, except for computed tomography (CT), there was a positive correlation between the number of primary descriptions and the number of radiologists. There was a trend towards shorter description times for magnetic resonance imaging (MRI), fluorography, densitometry, and radiography. An audit among MRC employees showed a significantly higher number of fully compliant reports without discrepancies, lower number of general comments, and clinically significant errors compared to the audit of on-site radiologists at medical institutions.</p></sec><sec><title>Research limitations</title><p>Research limitations: no data on (i) anatomical regions of the examinations, (ii) the balance of “normal/pathological” conclusion classes, (iii) the number of examinations with contrast enhancement, making establishing the changes in reporting time impossible.</p></sec><sec><title>Conclusion</title><p>Conclusion. The unification of diagnostic radiology at the MRC has improved report quality and reduced time costs.</p><p>Compliance with ethical standards.
