Для большинства современных производств характерно одновременное воздействие шума с другими, усугубляющими его действие, факторами.</p><p>Цель исследования — определить особенности профессиональной потери слуха при комбинированном воздействии шума и вибрации.</p></sec><sec><title>Материал и методы</title><p>Материал и методы. Проведена сравнительная оценка уровня профессионального риска и аудиометрических показателей в 3 группах работников: 1-я группа (n = 210) — подвергалась воздействию шума, 2-я (n = 170) — шума и локальной вибрации, 3-я (n = 200 человек) — шума и общей вибрации.</p></sec><sec><title>Результаты</title><p>Результаты. У работников 1-й группы профессиональная потеря слуха диагностирована достоверно (p &lt; 0,05) реже по сравнению с рабочими 2-й и 3-й групп (7% против 14 и 13% соответственно). Сочетанное воздействие шума и вибрации повышает риск профессиональной потери слуха в 1,82–1,85 раза (RR = 1,82–1,85; EF = 45–46%). Выявлены аудиологические особенности поражения слухового анализатора в зависимости от наличия и характера воздействующей вибрации, проявляющиеся изменениями порогов слуха в определённых спектрах частот и их комбинацией. По данным речевой аудиометрии выявлено нарушение разборчивости речи с наличием рече-тональной диссоциации у рабочих 2-й и 3-й групп, свидетельствующее о нарушениях в центральном отделе слухового анализатора.</p></sec><sec><title>Ограничение исследования</title><p>Ограничение исследования: изучение особенностей профессиональной потери слуха при комбинированном воздействии шума и вибрации у работников горнодобывающей промышленности по аудиометрическим показателям.</p></sec><sec><title>Заключение</title><p>Заключение. Проведённые исследования показали необходимость разработки индивидуальных программ сохранения слуха с учётом степени риска и выявленных особенностей формирования профессиональной потери слуха. Изучение воздействия шума и вибрации на потерю слуха требует углублённых исследований с учётом интенсивности и продолжительности воздействия вибрации, вида виброгенерирующего оборудования.</p><p>Соблюдение этических стандартов. Исследования проведены с соблюдением требований конфиденциальности персональных данных, этических норм и принципов проведения медицинских исследований с участием человека, изложенных в Хельсинкской декларации Всемирной медицинской ассоциации (2013 г.). Обследованные работники дали добровольное информированное согласие на участие в исследовании. Программа исследования одобрена Комитетом по биомедицинской этике ФБУН «Федеральный научный центр гигиены им. Ф.Ф. Исследование не имело спонсорской поддержки.</p></sec><sec><title>Поступила 10</title><p>Поступила 10.09.2023Принята в печать 11.10.2023Опубликована 23.12.2023</p></sec></abstract><trans-abstract xml:lang="en"><p>For most modern industries, there is characteristic the simultaneous impact of noise with other factors aggravating its effect.</p><p>The purpose of the study is to determine the features of occupational hearing loss in the combined effects of noise and vibration.</p><sec><title>Materials and methods</title><p>Materials and methods. A comparative assessment of the level of occupational risk and audiometric indicators was carried out in three groups of employees: 1st group (210 people) — is exposed to noise, 2nd group (170 people) — to noise and local vibration, 3rd group (200 people) — to noise and general vibration.</p></sec><sec><title>Results</title><p>Results. In group 1 workers exposed to isolated noise, occupational hearing loss was diagnosed significantly less frequently compared to group 2 and 3 including workers exposed to combined noise and vibration (7% vs. 14 and 13%, respectively, p &lt; 0.05). Combined exposure to noise and vibration increases the risk of occupational hearing loss by 1.82–1.85 times (RR = 1.82–1.85; EF = 45–46%). Audiological features of the lesion of the auditory analyzer have been revealed depending on the presence and nature of the affecting vibration, manifested by changes in hearing thresholds in certain frequency spectra and their combination. According to the data of speech audiometry, a violation of speech intelligibility with the presence of speech-tonal dissociation was revealed in workers of groups 2 and 3, indicating violations in the central department of the auditory analyzer.</p></sec><sec><title>Limitations of the study</title><p>Limitations of the study. The study is limited to the study of the features of occupational hearing loss in the combined effects of noise and vibration in mining workers according to audiometric indicators.</p></sec><sec><title>Conclusion</title><p>Conclusion. The conducted studies have shown the need to develop individual programs for the preservation of hearing, taking into account the degree of risk and the identified features of the formation of occupational hearing loss. The topic of research on the effects of noise and vibration on hearing loss requires in-depth research, taking into account the intensity and duration of vibration exposure, the type of vibration generating equipment.</p><p>Compliance with ethical standards. The studies were conducted in compliance with the requirements of confidentiality of personal data, ethical standards and principles of conducting medical research with human participation, set out in the Helsinki Declaration of the World Medical Association (ed. 2013).
