Сегодня медицинская лаборатория ориентирована на операционную эффективность и контроль затрат. В нашей работе представлен опыт преобразования лабораторной практики, её информатизации и реорганизации управленческой модели.</p></sec><sec><title>Цель исследования</title><p>Цель исследования. Создание управленческой модели быстрого, экономичного и высококачественного тестирования в лабораториях разного уровня для обеспечения в полном объёме москвичей услугами лабораторного сервиса.</p></sec><sec><title>Материал и методы</title><p>Материал и методы. Оценка ресурсного обеспечения клинико-диагностических лабораторий (КДЛ) Департамента здравоохранения города Москвы (ДЗМ), количества прикреплённого населения и потребности в тестах позволила: разработать и реализовать концепции по их централизации в амбулаторно-поликлиническом звене; унифицировать номенклатуру лабораторных исследований и тестов, создать единый справочник лабораторных исследований (ЕСЛИ) для централизованного лабораторного сервиса единой медицинской информационно-аналитической системы (ЕМИАС), разработать план развития системы управления качеством на основе международных стандартов и начать работы по стандартизации лабораторных услуг через аккредитацию медицинских лабораторий в национальной системе аккредитации; организовать централизованный широкомасштабный скрининг на SARS-CoV-2; внедрить технологию секвенирования для геномного надзора.</p></sec><sec><title>Результаты</title><p>Результаты. Лабораторная служба ДЗМ выстроена по 3-уровневой системе. Функционирует комплексный ЕСЛИ ЕМИАС. Лабораторные исследования информатизированы, выполняется передача результатов тестов в личный кабинет и электронную медицинскую карту пациента. Создано единое информационное пространство для всех КДЛ ДЗМ. Изменены тарифы на лабораторные услуги реестра Московского городского фонда обязательного медицинского страхования, оптимизирован перечень. Под контролем трат возросла производительность лабораторий. Удовлетворены потребности Москвы в лабораторных услугах.</p></sec><sec><title>Ограничения исследования</title><p>Ограничения исследования. Сценарий организационных подходов проходил при стратегическом партнёрстве с ДЗМ и доказал состоятельность только в условиях города Москвы.</p></sec><sec><title>Заключение</title><p>Заключение. Решены задачи точной медицины и обеспечено устойчивое предоставление лабораторных услуг населению КДЛ ДЗМ.</p><p>Соблюдение этических стандартов. Исследование одобрено на заседании Научно-методического совета ГБУ «НИИ ОЗММ ДЗМ» г. Москвы, протокол № 5 от 17.05.2022, материалы выполненного исследования отвечают требованиям Этического кодекса проведения исследований в сфере организации здравоохранения и общественного здоровья, разработанного в ГБУ «НИИ ОЗММ ДЗМ», по соблюдению этических норм в процессе подготовки исследования, соблюдению прав человека в отношении респондентов и самих исследователей, соответствию социального портрета и необходимой подготовки интервьюеров.</p></sec><sec><title>Участие авторов</title><p>Участие авторов: Цибин А.Н. — концепция и дизайн исследования; Латыпова М.Ф. — сбор и обработка материала, написание текста; Комаров А.Г. — написание текста; Слуцкий Е.А. — сбор и обработка материала; Иванушкина О.И. — составление списка литературы. Today, the medical laboratory is focused on operational efficiency and cost control. Our work presents the experience of transforming laboratory practice, its informatization and reorganization of the management model.</p></sec><sec><title>Purpose of the study</title><p>Purpose of the study. Creation of a management model for fast, economical and high-quality testing in laboratories of various levels to provide Muscovites with laboratory services in full.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. An assessment of the resource support of clinical diagnostic laboratories (CDL) of the Moscow City Health Department (MCHD) number of attached population and the need for tests, made it possible to develop and implement concepts for their centralization in the outpatient clinic link, unify the nomenclature of laboratory studies and tests, create a unified reference book of laboratory studies (IF) for the centralized laboratory service (CLS) of the unified medical information and analytical system (UMIAS), develop a plan for the development of a quality management system based on international standards and begin the activity on the standardization of laboratory services through accreditation of medical laboratories in the national accreditation system, manage centralized large-scale screening for SARS-CoV-2; implement sequencing technology (NGS) for genomic surveillance.</p></sec><sec><title>Results</title><p>Results. The MCHD laboratory service is built according to a 3-level system. There is a complex IF EMIAS. Laboratory studies are computerized, test results are transferred to the personal account and the patient’s electronic medical record. A single information space has been created for all CDL MCHD. The tariffs for laboratory services of the registry of the Moscow City Compulsory Medical Insurance Fund (MCCMIF) have been changed, the list has been optimized. Under the control of spending, the productivity of laboratories has increased. Moscow’s needs for laboratory services have been met.</p></sec><sec><title>Research limitations</title><p>Research limitations. The scenario of managerial approaches was carried out in a strategic partnership with MCHD, proved to be viable only in the conditions of the city of Moscow.</p></sec><sec><title>Conclusion</title><p>Conclusion. The problems of precision medicine have been solved and the sustainable provision of laboratory services to the population by the clinical diagnostic laboratories of the MCHD has been ensured.</p><p>Compliance with ethical standards.
